If you are a member of a plan not included on the above list and have PPO insurance, you have two options:

1. If the service is $75 or less you pay that amount in full at the time of service. If the service total is more than $75, you pay 20%. We will bill the insurance and if are we are paid in full, we will reimburse you.

2. You may pay in full and submit the bill to your insurance for reimbursement. (We can not guarantee they will reimburse you.)
